01 / PREVENTIVE PROPHYLAXIS
2 Healthy Mouth Cleanings & 2 Preventive Exams
Includes two standard dental cleanings (adult prophylaxis) and two comprehensive periodic exams per plan year. Our dentists meticulously evaluate soft tissue, check bite stability, and perform systematic oral cancer screenings at each visit.
CLINICAL NOTE: CLEANING TYPE MUST MATCH GUM HEALTH
In the presence of active periodontal disease, specialized periodontal maintenance or deep scaling is clinically required by dental standards. Standard prophylaxis cannot be substituted for active periodontal therapy.

Dental X-Rays & 2 Fluoride Applications

The Healthy Mouth Membership Plan is an exclusive direct agreement between you and Northwest Portland Dental. It is structured intentionally for complete administrative transparency.
WHAT MEMBERSHIP IS NOT
- Not Insurance: There are no claims, deductibles, waiting periods, pre-authorizations, or annual maximum limits.
- Not Financing: This is a direct prepaid clinical care arrangement, not a third-party credit line or debt product.
- Not an In-House Payment Plan: Total annual fees are paid upfront to guarantee immediate schedule availability and locked savings.
$450 Prepaid Annually
Total membership fee of $450 is collected in full at the time of enrollment, immediately activating full preventive coverage and treatment fee reductions.
12-Month Participation Term
Your agreement is effective for 365 consecutive days starting on the exact date of enrollment. All included cleanings and exams must be received within this active window.
Uninsured Patient Eligibility Only
Participants must not have active dental insurance. The program cannot be combined with, billed to, or secondary to third-party commercial insurance, Medicare, or Medicaid.
Non-Refundable & Non-Transferable
Annual dues are non-refundable once clinical services or discounts have been accessed. Benefits and discounts are assigned solely to the registered individual and cannot be transferred.
Exclusive In-Office Application
Plan privileges apply solely to dental care provided directly at Northwest Portland Dental. Outside specialist referrals, hospital visits, or external lab fees are not eligible for member savings.

Who is eligible to participate in the Healthy Mouth Membership Plan?
This plan is designed specifically for uninsured patients. If you currently have active commercial or private dental insurance, you cannot enroll due to contractual and billing regulations. All new and existing uninsured patients are immediately eligible.
What if I require periodontal maintenance instead of standard prophylaxis?
Standard membership covers two routine preventive cleanings (prophylaxis). For patients diagnosed with active periodontal disease, the plan credits the full standard preventive allowance toward your specialized periodontal therapy, with remaining periodontal maintenance visits discounted at your tiered member rate.
Are treatments completed by outside specialists covered under the plan?
No. The Healthy Mouth Membership Plan applies exclusively to professional dental services performed directly in our Northwest Portland practice. Procedures requiring referral to outside endodontists, oral surgeons, or periodontists are billed separately by those independent practices.
Can membership discounts be combined with CareCredit or external financing?
Yes, CareCredit may be utilized for plan-covered procedures. However, because third-party merchant financing carries administrative processing fees, the member discount applied when using extended financing options is adjusted accordingly at the time of treatment calculation.
How does the annual membership renewal and coverage period operate?
Your membership remains active for exactly 365 days starting from your date of enrollment. All included cleanings, examinations, and diagnostic x-rays must be scheduled and completed within that 12-month period. Unused services do not roll over into subsequent membership years.
Can I use this membership alongside secondary or supplemental dental insurance?
No. The membership plan cannot be combined with, billed alongside, or coordinated as secondary coverage with any private, commercial, or state dental insurance plan. It functions purely as a direct in-office dental care agreement.
Are there any waiting periods or annual maximum benefit caps?
There are zero waiting periods and no annual maximum dollar caps. Your 100% preventive coverage and reduced treatment rates take effect immediately upon enrollment payment, with no cumulative cap on your annual savings.
